Purchased from the Anna Bé bridal boutique in Denver. The dress is by Haley Paige, has a deep v in the front with mesh and lace, and a flowing though not quite aline skirt with a hidden slit in the front.
The dress has been altered, but is probably close to a size 12/10 at the moment.
